Lakewood Housing Authority will be issuing Interest Cards for the Combined Waiting List on:

Tuesday, January 22, 2008 8:00 am to 4:00pm
Wednesday,January 23,2008 8:00 am to 4:00 pm

Applicants must be 18 years of age or older.

Combined waiting list interest cards must be picked up at the City of Lakewood, Civic Center South, 480 South Allison Parkway, Lakewood, Colorado in the main lobby area on January 22 and January 23, 2008.

Completed cards must be:

Returned by mail with proper postage applied by applicant.
Filled out completely and correctly.
Signed and dated by applicant.
Postmarked by the deadline date of February 6, 2008.

HAND DELIVERED CARDS WILL NOT BE ACCEPTED.

Completed interest cards will be date and time stamped and added to the waiting list upon return to the Lakewood Housing Authority.

INCOMPLETE CARDS AND CARDS POSTMARKED AFTER THE DEADLINE DATE WILL BE DESTROYED AND WILL NOT BE ADDED TO THE WAITING LIST.

Any applicant with a disability has an opportunity to request a reasonable accommodation at any time. The LHA will provide auxiliary aids and reasonable accommodation as required for effective communication and participation in its programs.
